You can download the Access Request Form on the NDIS website. You can also call or email the NDIS to send you the Access Request Form. Complete section 1 of the request form and attached the consent form.

  1. Get Evidence

One of the most important pieces of evidence that you need to prove in your application is your identity. They might ask you to submit shreds of evidence of your age, citizenship, residency status in Australia, and your disability. Do not send the pieces of evidence on a USB or a CD. You should create a folder for them on your computer.

  1. Send your Access Request form and pieces of evidence

You can send them via email at NAT@ndis.gov.au. You can also send them by mail. So you can send them to GPO Box 700, Canberra ACT 2601. You can also submit them to the nearest local NDIA in your area.
